| 12 | Student’s Pre-School Check-Up Record from the Primary Health Care Center (PHCC), along with a certificate stating “FIT TO JOIN SCHOOL” |
| 13 | Copy of the Student’s Vaccination Card (for KG-1 to Class 1) |
| 14 | School Leaving Certificate and Copy of Previous Result Card (both must be attested by MOFA in Pakistan, Pakistan Embassy in Qatar, and MOFA in Qatar) |
